Ultrasound

An abdominal ultrasound for a dog or cat costs roughly $300 to $900, depending on whether a general practitioner or a board-certified specialist performs it, whether sedation is needed, and whether samples are taken during the scan. It uses high-frequency sound waves to build a live image of soft tissue, with no radiation involved.

The question that brings most owners here is why they are being charged for an ultrasound after already paying for x-rays.

X-ray and ultrasound answer different questions

They are not two versions of the same test, and neither replaces the other.

X-rays show density and outline across the whole area at once. They are the right tool for bones, gas patterns, foreign objects, the silhouette of the heart, and the general size, shape and position of the abdominal organs. What they cannot do is show what is happening inside an organ — a liver on a radiograph is a grey shape.

Ultrasound shows internal architecture in real time. It distinguishes the layers of the intestinal wall, finds a mass inside the spleen, measures how thick the wall of the bladder is, watches blood flow, sees fluid an x-ray would miss, and — decisively — lets a needle be guided into a specific spot to take a sample. What it cannot do is see through gas or bone: a gas-filled gut hides everything behind it, and the lungs and skull are largely off-limits.

So a typical workup is: radiographs show the spleen is enlarged; ultrasound shows there is a mass within it, whether it has ruptured, and whether the liver looks involved. Two answers, two tests. That sequence is normal and is not double-billing.

In an emergency, a fast scan is the fastest answer

A focused scan for free fluid — often called an AFAST — takes a few minutes and asks one question: is there blood or fluid where it should not be? For a collapsed older large-breed dog with pale gums, this finds internal bleeding from a ruptured splenic mass faster than anything else available, and it costs far less than a full study. It is a triage tool, not a diagnosis, and a full scan usually follows once the patient is stable.

Why your pet gets shaved

Ultrasound cannot cross air. Any air between the probe and the skin blocks the image completely, and hair traps air against the skin no matter how much gel is applied.

So the belly gets clipped — and the patch is often larger than owners expect, because a proper abdominal study covers everything from below the ribs to the pelvis. The hair grows back over a few weeks. It is worth knowing in advance simply because the surprise is a common source of upset, particularly with show animals or long-coated breeds, where telling the practice beforehand may allow a smaller area to be planned.

Sedation

Many pets tolerate ultrasound with gentle restraint on a padded table — there is no pain involved, just the need to lie still, often on their back, for 30–60 minutes. Sedation is used for anxious animals, those with a painful abdomen, and whenever a needle sample is being taken, where an unexpected movement matters.

Who performs the scan matters more than the machine

This is the least-known and most consequential thing about ultrasound: it is operator-dependent to a degree that other imaging is not.

A radiograph is a fixed image that any reviewer can examine later. An ultrasound is a live examination — the person holding the probe decides where to look, what angle to use, and what is worth capturing. Anything they do not look at simply does not exist in the record.

The result is that the same machine in different hands produces genuinely different quality. A board-certified veterinary radiologist or an experienced internal medicine specialist will find things a brief screening scan misses. It is entirely reasonable to ask who will be performing the study and whether the images will be reviewed by a specialist — and if the finding will drive a major decision, such as whether to pursue surgery, that is when it is worth seeking out a specialist scan.

Can ultrasound detect cancer?

It can find masses, abnormal organ texture, enlarged lymph nodes, and fluid — all of which raise or lower suspicion. It cannot diagnose cancer.

The diagnosis comes from cells or tissue: a fine-needle aspirate or biopsy, frequently taken under ultrasound guidance during the same session, then examined under a microscope. An ultrasound report saying a splenic mass is present does not say whether it is a benign haematoma or a haemangiosarcoma — and those two have completely different outcomes. Anyone who tells you an image alone settled it has overstated what the test does.

Who isn't a candidate

  • Other: Not a contraindication but a hard physical limit — ultrasound cannot see through gas or bone. A gas-filled gut obscures what lies beneath it, and the lungs and brain are largely inaccessible, which is why chest disease usually needs radiographs or CT instead.
  • Concurrent condition: An unstable patient should have a brief focused scan for free fluid rather than a full study; a complete abdominal ultrasound takes 30-60 minutes of lying still.
  • Other: Ultrasound-guided sampling is contraindicated with uncorrected clotting disorders, and aspirating a suspected haemangiosarcoma or a distended bladder carries specific risks the sonographer weighs case by case.
