Nearly everything written about this online is published by a company that sells it. This page is not, so here is the version with the caveats left in.
What it is
The common veterinary model is autologous adipose-derived: fat is collected from your dog under sedation or anaesthesia — usually from behind the shoulder or the abdomen — processed to concentrate the mesenchymal stromal cells it contains, and injected back into the affected joints, sometimes with an intravenous dose as well. Processing happens either at a laboratory (a few days' turnaround) or in-clinic on the same day.
Mesenchymal cells do not, in this context, grow new cartilage. Their effect is understood to be largely anti-inflammatory and immune-modulating — they change the chemical environment inside an arthritic joint. That distinction matters, because "regenerative" implies regrowth and regrowth is not what the evidence describes.
What the evidence actually shows
Honestly stated: promising, not settled.
Several controlled studies in dogs with osteoarthritis of the hips or elbows have found statistically significant improvements in lameness scores, owner-assessed pain and range of motion compared with placebo, generally over three to six months. That is real and it is more than most complementary therapies can show.
The important limitations, which you will rarely see on a clinic's own page:
- The studies are small. Dozens of dogs, not hundreds.
- Protocols and products differ substantially between providers — cell source, dose, processing method, whether cells are expanded in culture. A result from one product does not transfer to another.
- Follow-up is mostly short. Durability past a year is not well established.
- There is no approved, standardised veterinary product in the way there is for a licensed drug, so quality control varies with the provider.
None of that means it does not work. It means the right question is not "does stem cell therapy work" but "what specifically is this expected to do for my dog, over what period, and what happens if it does not?" Ask it, and judge the answer.
What it does not replace
Weight is the intervention with the strongest evidence base in canine osteoarthritis, and it costs nothing. Overweight dogs improve measurably on weight loss alone, and no injection compensates for carrying an extra 20%.
A complete arthritis plan is multimodal:
- Weight management — the largest single effect.
- Controlled exercise and [hydrotherapy](/procedures/hydrotherapy/) — maintaining muscle is protective.
- NSAIDs, and newer options such as monoclonal antibody injections for pain.
- Joint supplements and omega-3 fatty acids — modest but real.
- [Laser therapy](/procedures/laser-therapy/) and acupuncture as adjuncts.
- Surgery where the joint is beyond medical management.
Stem cell therapy belongs alongside these, not instead of them. Any provider who positions it as a replacement for weight control or pain medication is overselling.
Stem cells versus PRP
You may be offered platelet-rich plasma instead, and often it is what people mean when they say "regenerative medicine".
PRP is made from a blood sample spun down in-clinic, takes under an hour, needs no fat collection, and costs $300 to $900 per treatment. Stem cell therapy requires a surgical fat collection, laboratory processing, and several times the price.
Both aim at the same target. PRP has a shorter duration of effect in most reports; stem cell therapy is claimed to last longer. Given the cost difference, starting with PRP is a defensible choice, and a provider who dismisses it out of hand is worth questioning.
Who should not have it
- Any dog with active cancer or a history of it. The effect of administered stem cells on tumour behaviour is not established, and this is a standard exclusion.
- Active infection, in the joint or at the collection site.
- End-stage joints where the cartilage is essentially gone. An injection into bone-on-bone will not help, and the honest recommendation is a joint replacement or a salvage procedure.
- Dogs unfit for anaesthesia, since collection and injection usually require two events.
What to ask before you pay
1. What has this achieved in your own patients with my dog's condition and stage? 2. Which product or system do you use, and what published evidence supports it? 3. What does the total cost cover — collection, processing, injection, rechecks, banking? 4. How long before we expect a change, and how will we measure it? A gait analysis or a validated owner questionnaire beats an impression. 5. What is the plan if there is no improvement at three months?
A provider who answers these specifically is worth trusting. One who answers with testimonials is not.
Who isn't a candidate
- Other: Active cancer, or a history of it — the effect of administered stem cells on tumour behaviour is not established, and most protocols exclude these patients.
- Other: Active infection, including infected joints or skin at the collection or injection site.
