If you are reading this while your dog is in the back, the short version is: this is a well-established emergency procedure, it usually works quickly, and the more important question is what caused the fluid.
Why it is an emergency
The heart sits inside a tough fibrous sac. That sac does not stretch quickly.
When fluid accumulates in it, pressure rises and squeezes the heart from the outside. The heart cannot fill properly, so it cannot pump properly. This is cardiac tamponade, and it is why a dog with pericardial effusion collapses.
Draining even a modest amount of fluid relieves the pressure immediately. Few interventions in veterinary medicine produce such a fast, visible improvement — a dog that was grey and collapsed can be sitting up within minutes.
What it looks like at home
- Sudden collapse or fainting
- Very pale or grey gums
- A swollen, fluid-filled belly developing over days, as blood backs up
- Marked lethargy and exercise intolerance appearing over hours to days
- Fast, shallow breathing, often unwilling to lie down
Older large-breed dogs, particularly Golden Retrievers and German Shepherds, are over-represented.
The procedure
A needle or small catheter is passed through the chest wall into the pericardial sac, guided by ultrasound, usually with sedation and local anaesthetic. Fluid is drawn off — often a surprisingly large volume, and usually dark red.
It is monitored on ECG throughout, because contact with the heart can trigger rhythm disturbances. Risks are real but modest in experienced hands: bleeding, arrhythmias, and lung puncture.
The scan that decides everything
Ask for the echocardiogram to be done while the fluid is still present.
This is the single most useful thing an owner can know. Fluid outlines a mass; drain it first and a small tumour at the heart base can become very hard to see. The scan performed before or during drainage is what identifies the cause, and the cause is what determines everything that follows.
The three causes, and why they matter
Haemangiosarcoma of the right atrium. An aggressive cancer, and unfortunately the most common cause in the classic older large-breed patient. The outlook is poor, typically measured in months even with treatment.
Chemodectoma at the heart base. Slow-growing and locally invasive rather than aggressively spreading. Dogs often do well for a long time, and pericardial surgery can give good palliation.
Idiopathic pericardial effusion. No cause found — a diagnosis reached by excluding the others. This one carries the best news: it may recur once or twice and can then be cured outright by pericardiectomy, removing part of the sac so fluid can never build pressure again.
Brachycephalic breeds and younger dogs are more likely to fall into that last group.
It usually comes back
Drainage treats the tamponade, not the cause. Recurrence is the norm rather than the exception.
A dog that has needed draining more than once should be assessed for pericardiectomy — the surgery that removes part of the sac and prevents tamponade permanently. For idiopathic disease this is close to curative, and it is worth raising early rather than after the third emergency.
Cats
Different picture. Pericardial effusion in cats is usually secondary to heart failure or FIP rather than a primary cardiac tumour, and tamponade is less common. The underlying disease drives both treatment and outlook.

Who isn't a candidate
- Other: A small effusion that is not compressing the heart — draining it adds risk without benefit, and monitoring is the correct answer.
- Concurrent condition: Significant clotting disorder, where a needle passed near the heart carries a high risk of uncontrolled bleeding; correct the coagulopathy first where time allows.
- Other: Suspected left atrial rupture from advanced mitral valve disease — draining can worsen bleeding, and the decision belongs to a clinician with ultrasound in hand.
