The distinction that decides everything

Dialysis is a bridge, not a treatment. It replaces kidney function temporarily while something else changes. What matters is whether anything else can change.

Acute kidney injury — where dialysis is transformative. The kidneys have been suddenly poisoned or damaged, but the tissue can recover if the animal survives the days or weeks it takes. Dialysis buys exactly that time, and it converts a fatal situation into a survivable one. This is the situation dialysis exists for.

End-stage chronic kidney disease — where it does not. The kidneys have been failing gradually over months or years and will not recover. Dialysis makes the patient feel better for a day or two at a time, then the numbers climb again. Without a transplant to bridge toward — and feline kidney transplantation is available at very few centres, with its own substantial ethical and practical requirements — it delays an outcome it cannot change.

An owner asking "can my seventeen-year-old cat with chronic kidney disease have dialysis" is usually asking about the second situation. The kind answer is the honest one: technically yes, and it would not give them their cat back.

What acute kidney injury looks like

The most common causes are treatable, and time is everything:

  • Antifreeze (ethylene glycol). Sweet-tasting, rapidly fatal, and there is an antidote that only works early. Hours matter.
  • Lilies, in cats. Every part of the plant — petals, leaves, pollen, even the vase water. A cat that chews a lily or grooms pollen from its coat needs an emergency vet immediately, before any signs appear.
  • Grapes and raisins, in dogs, unpredictably.
  • NSAID overdose — including human ibuprofen and naproxen, and veterinary NSAIDs given at the wrong dose.
  • Leptospirosis, a bacterial infection that is vaccine-preventable in dogs.
  • Urinary obstruction left too long.
  • Severe dehydration, shock, sepsis or heatstroke.

The signs: producing little or no urine, vomiting, profound lethargy, loss of appetite, a chemical or ammonia smell on the breath, and in severe cases seizures or collapse.

If you know or suspect an exposure, go now and take the packaging or the plant with you. Do not wait for signs. For several of these, treatment started in the first hours prevents kidney injury entirely, and no dialysis is ever needed.

What it involves

Intermittent hemodialysis circulates blood through a machine that filters waste across a membrane and returns it — sessions of several hours, repeated over days.

Continuous renal replacement therapy (CRRT) does the same more gently and continuously over 24 hours or more, better tolerated by unstable patients.

Both need a large dialysis catheter placed in the jugular vein, continuous anticoagulation of the circuit, and intensive nursing throughout. The patient is hospitalised in a critical care unit for the duration.

Availability is genuinely limited. A handful of university teaching hospitals and large referral centres offer it, and geography may well decide the question for you.

Risks

  • Bleeding, from the anticoagulation the circuit requires
  • Catheter complications — infection, clotting, damage to the vein
  • Fluid and electrolyte shifts, particularly during the first sessions
  • Low blood pressure during treatment
  • Very small patients may not be suitable for some machines, since the volume held in the circuit is too large a fraction of their blood volume

Outcomes

For acute kidney injury, the honest figure is that roughly half of treated patients survive to discharge, and among survivors a substantial proportion regain enough function to live normally, though some are left with residual chronic kidney disease.

That number sounds sobering until you set it against the alternative: without dialysis, an animal producing no urine at all has a very poor outlook indeed.

The outlook is better where the cause is identified and reversible, where treatment started early, and where the patient is otherwise healthy.

The cost conversation, plainly

This is expensive, it is often not a single session, and the total is difficult to predict at the outset because it depends on how the kidneys respond.

Ask for a realistic range and a decision point — how many days before you will know whether the kidneys are recovering. A good critical care team will give you that, and will tell you when continuing is no longer in the patient's interest.

Declining is a legitimate choice. Aggressive supportive care with fluids and medication remains an option and helps some patients. So does euthanasia, for an animal that is suffering with no realistic path back. Nobody should feel they have failed their pet for reaching a financial or practical limit — and a team that makes you feel that way is not doing its job.

Who isn't a candidate

  • Other: End-stage chronic kidney disease with no transplant plan — dialysis buys time for kidneys that can recover, and where nothing will change it prolongs dying rather than living.
  • Weight: Very small patients, where the volume held in the circuit is too large a fraction of the animal's blood volume for some machines.
  • Concurrent condition: Uncontrolled bleeding or a coagulation disorder, since the circuit requires anticoagulation.
