Neutering a cryptorchid dog — one with an undescended testicle — costs roughly $500 to $1,800, against $150 to $800 for a routine neuter.

It is not a neuter with an extra step
A routine neuter never enters the abdomen. A small incision in front of the scrotum, both testicles removed through it, done in twenty minutes.
A cryptorchid neuter is abdominal surgery. The retained testicle may be anywhere along the path it failed to complete — high near the kidney, low near the bladder, or stuck in the inguinal canal in the groin. Finding it means opening the abdomen and searching, which brings:
- A longer general anaesthetic
- More surgical time and a second surgical site
- An abdominal incision that must be closed in layers
- A proper two-week recovery rather than a few days
That is the price difference. It is not a markup.
Where the testicle sits also matters to the quote. Inguinal retention, in the groin, is simpler and cheaper than abdominal retention. Your vet may not know which until they examine or image your dog.
Why it has to come out
The case is strong, and worth understanding rather than accepting on trust.
A testicle is meant to sit in the scrotum, several degrees cooler than body temperature. One retained inside the body sits permanently at core temperature, and that changes it.
Cancer risk rises substantially. Sertoli cell tumours and seminomas are markedly more common in retained testicles than in descended ones.
Torsion. A retained testicle can twist on its blood supply, causing sudden severe abdominal pain, vomiting and collapse — a surgical emergency.
Hormonal consequences. Sertoli cell tumours can produce oestrogen, causing symmetrical hair loss, enlarged nipples, a pendulous prepuce, and attraction of other male dogs. In the worst cases the oestrogen suppresses the bone marrow, which is life-threatening and difficult to reverse. This last consequence is not widely known among owners and is a genuine reason not to wait.
Removing only the descended one is a mistake
It occasionally gets suggested as a cheaper compromise. It is not a compromise, it is the wrong operation — the retained testicle is the one carrying the risk. Both must come out.
Timing
Most testicles that are going to descend do so by around eight weeks of age. After about six months, descent is very unlikely.
So: do not rush to operate on a young puppy whose testicle may still arrive, and do not wait past a year, because waiting offers nothing while risk accumulates.
It is hereditary
Cryptorchidism is inherited and sex-limited — carried by both parents, expressed only in males. Affected dogs should not be bred, and neither, ideally, should their close relatives.
Over-represented breeds include Yorkshire Terriers, Chihuahuas, Miniature Poodles, Pomeranians, Boxers and German Shepherds.
Finding it
Your vet will palpate the groin first, since an inguinal testicle can often be felt. Ultrasound locates most abdominal ones beforehand, which helps plan the approach and refine the quote.
Laparoscopic removal is offered at some centres — smaller incisions, less pain, faster recovery, at a higher price.
Cats too
Far less common, but it happens. The usual clue is a cat that still sprays, still has the tomcat face and behaviour, and is still very much intact months after a neuter. The characteristic barbs on the penis are testosterone-dependent and their presence is a good indication that testicular tissue remains somewhere.
Recovery
Longer than a routine neuter, because there is an abdominal incision.
- A cone or surgical suit for 10 to 14 days
- Lead walks only, no running or jumping, for two weeks
- Sutures out at 10 to 14 days
- Watch for a swollen, hot or discharging incision, repeated vomiting, or refusal to eat
Who isn't a candidate
- Age: Before about six months of age, when a testicle may still descend on its own; most vets wait rather than operate on a puppy who would have resolved without surgery.
- Concurrent condition: Uncontrolled illness or infection — as with any elective surgery, the patient is stabilised first.
- Other: Removing only the descended testicle and leaving the retained one. This is not a shortcut, it is a mistake: the retained testicle carries the cancer risk and must come out.
