A blood transfusion for a dog or cat costs roughly $400 to $1,500 per unit, including typing, crossmatching, administration, and the monitoring that goes with it. Most owners meet this number in an emergency room at an hour when they are least equipped to evaluate it.
So the most useful thing to say first is what a transfusion actually is: it buys time. It does not treat the disease that caused the anaemia. It restores the blood's ability to carry oxygen while the underlying problem is diagnosed and — if possible — stopped. Whether your pet recovers depends far more on what caused the blood loss than on the transfusion itself.
Why a transfusion is being recommended
Three broad situations lead here.
Blood loss. Trauma, a bleeding tumour (a ruptured splenic mass is the classic one in older large-breed dogs), surgical haemorrhage, or severe gastrointestinal bleeding. Anticoagulant rodenticide poisoning also belongs here — a dog that ate rat bait may show nothing for several days and then bleed catastrophically.
Blood destruction. In immune-mediated haemolytic anaemia the immune system attacks the animal's own red cells. Certain toxins and infections do the same.
Failure to produce blood. Chronic kidney disease, bone marrow disease, and some cancers reduce red cell production over weeks to months, which is why a pet can be profoundly anaemic and still walking around — the body adapts slowly.
Blood types are real, and cats are the strict case
Dogs have several blood group antigens, of which DEA 1 matters most. Dogs do not usually carry pre-formed antibodies against types they have never met, so a first transfusion is relatively safe even before typing. That tolerance ends immediately afterwards: a dog transfused more than about four days ago has been sensitised, and any subsequent transfusion requires crossmatching.
Cats are different, and this is the part that surprises people. Cats have types A, B, and the rare AB, and they carry naturally occurring antibodies against the type they do not have — with no prior exposure. A type-B cat given type-A blood can suffer a severe, sometimes fatal reaction on the first transfusion. Type B is uncommon overall but much more frequent in some breeds, including British Shorthairs, Devon Rex and Ragdolls.
This is why blood typing a cat is never an optional add-on. It takes minutes and it is the difference between a treatment and a serious hazard.
Where veterinary blood comes from
There is no equivalent of a national blood service for animals, and this is the question owners most often ask.
Blood comes from volunteer donor pets — typically large, calm, healthy adult dogs and cats whose owners bring them in on a schedule, much like human donation — and from commercial veterinary blood banks that supply hospitals, plus in some practices a resident donor animal. Donors are screened for infectious disease and blood-typed. Donation is voluntary, monitored, and the donor goes home the same day.
Supply is genuinely limited, particularly feline blood and particularly out of hours. It is one reason costs are what they are, and occasionally a reason a hospital must transfer a patient.
Which product, and why it is not always "blood"
- Packed red blood cells — red cells with most plasma removed. The usual choice for anaemia.
- Fresh frozen plasma — clotting factors and proteins, no red cells. Used for rodenticide poisoning and clotting disorders. A pet that is bleeding because it cannot clot may need plasma rather than red cells.
- Whole blood — both, used for acute major haemorrhage.
- Platelet products — less widely available, for specific platelet disorders.
What happens during the transfusion
A catheter is placed and the product is given slowly through a filtered line. The first 15–30 minutes run at a deliberately reduced rate because that is when reactions appear. Temperature, pulse, respiration and blood pressure are checked repeatedly throughout. A typical transfusion takes 4 to 6 hours, and most patients are hospitalised for it and for a period afterwards.
Reaction signs — facial swelling, hives, vomiting, fever, tremors, or dark red-brown urine — mean the infusion is stopped immediately. Most reactions are mild and manageable when caught early, which is what the monitoring is for. Serious reactions are uncommon, particularly when typing and crossmatching have been done properly.
How long the effect lasts
Transfused red cells survive days to a few weeks, not indefinitely. If the underlying cause has been fixed — bleeding stopped, tumour removed, poisoning reversed — the pet's own marrow takes over and one transfusion may be all that is needed. If the cause is ongoing, such as active immune destruction or advanced kidney disease, further transfusions may be required, and at that point the conversation shifts from cost to what is achievable and kind.
That is a fair question to ask directly: if this works, what does the next month look like? A good emergency clinician will answer it honestly.
